A. Assess your ability to gather information on your client within the iHuman Virtual
Patient Encounter.
I have attached my first attempt and my last attempt to the assignment submission for
reference of this reflection. I believe I did well on my first attempt, but I was unfamiliar with the
software. I asked many questions during the history of present illness interview, but they were
not exactly the correct questions to ask. I asked 102 total questions, only 10 of the questions
were correct, and 11 questions were missed. The hardest part for me was deciding which
questions were appropriate and trying to focus on the main problem. I realized with my review at
the end I asked a lot of questions and was doing more of a comprehensive exam than a focused. I
documented the subjective findings in the EHR as well as in a word document. I also utilized a
SOAP note template which was helpful in documentation purposes. My physical exam score on
my first attempt was a 92% which I thought was a high score for my first try. I performed all the
correct exams, but some were not performed correctly. Performing the eye exams were hardest
for me because they are very particular on iHuman with how it is performed. I had to move very
slowly for the assessment of the extraocular eye muscles to assess for nystagmus. I documented
the findings in the EHR, as well as a word document, and the SOAP note template. On my last
attempt I feel like I understood the software much better and performed the assignment with less
stress. I scored a 100% on the history section. I asked 106 questions, and twenty-one of my
questions were correct. I also scored a 100% on the physical exam portion. After reviewing the
case study a few times, I realized what to do and not to do during the physical exam and how to
operate the system. I feel much more comfortable during the physical exam now than I did at the
beginning. For my next virtual encounter, I should go back through the physical exam to ensure I
have completed all steps and filled everything out appropriately, rather than moving onto the
